The current forms, including the September 2025 changes
The Personal Injuries Proceedings Regulation 2025 commenced on 1 September 2025 and brought new mandatory notice of claim forms, with separate forms for non-healthcare claims, healthcare claims and dependency claims. Precedent banks built before that date are out of date. Ours are not.
